Case Management Supervisor

Position Summary:

The Case Management Supervisor will provide clinical and administrative oversight of child and adult behavioral health case managers based in Androscoggin County. It is essential that case managers have a supportive supervisor and administrative structure on which to rely. Supervisors are integral in ensuring high-quality, collaborative and client-centered case management is offered in our local communities. As a program, we emphasize high quality supervision with an emphasis on ethics, case review and self-care.

Roles and Responsibilities (Include but are not limited to the following)

  • Provide administrative and clinical supervision to case managers and wellness coordinators working with adults and children.
  • Review, provided feedback on and approve clinical documentation and case review for purpose of quality and compliance.
  • Assist in the recruitment, hiring, orientation and training of new case managers and wellness coordinators.

Qualifications (Include but are not limited to the following):

  • Has independent master’s level license/ LCSW or LCPC preferred.

Privacy Officer

Position Summary:

As the Privacy Officer you will monitor the agencies compliance with HIPAA by identifying and documenting errors and areas for improvement and also provide education, training and communication about privacy policies to staff/vendors/affiliates.

Roles and Responsibilities (Include but are not limited to the following):

  • Chair the HIPAA Committee to develop and improve HIPAA compliance, identify needed updates to policy and procedure, coordinate agenda items and maintaining minutes for all meetings
  • Lead annual risk assessments and risk reduction strategies in collaboration with HIPAA Committee and IT; documenting findings, recommendations and steps needed to reduce risk exposures
  • Train and educate staff and vendors on privacy policies, explaining consequences
  • Ensure/Work with all departments to follow company privacy practices and implement needed changes
  • Ensure the document disposal procedures are established and followed by all departments
  • Log and respond to breaches and privacy violation problems, working to mitigate exposure
  • Interact with employees and affiliates at all levels on privacy issues to answer questions and provide guidance about privacy, regulations, laws, and reporting
  • Ensure a BAA/Confidentiality Agreement is obtained whenever necessary and confirm through due diligence their practices meet HIPAA requirements
  • Monitor affiliate HIPAA attestation compliance with affiliates
  • Ensure compliance and timely execution of record requests
  • Back up to reception
  • Other duties as assigned

Qualifications (Include but are not limited to the following):

  • A high school diploma or equivalent required; Associates or Bachelor’s Degree in the mental health, social service, or health field or equivalent experience preferred
  • Experience in a health care organization and/or with protected company information preferred
  • Experience and knowledge of HIPAA regulations preferred

School Based Clinician

Health Affiliates Maine is seeking Licensed Clinicians for year-round placements at a variety of elementary schools in central Maine.

Opportunities available:

  • Mechanic Falls– 1 day per week
  • Belgrade– 1 day per week
  • Freeport – 1/2-1 day per week
  • Steep Falls – 1/2 -1 days per week
  • Standish – 1-2 days in Standish for high school age

Summary:

This is a fee for service position. The clinician will receive referrals from staff, complete an assessment, and develop a treatment plan in conjunction with the client and/or family. Individual and/or family therapy will occur on site at the school. Successful candidates will need to meet all hiring requirements.

Mental Health Case Manager

Position Summary

Behavioral Health Home Coordinator – Sign-on Bonus!

Health Affiliates Maine is accepting resumes for per diem Behavioral Health Home (BHH) Wellness Coordinators to provide services to children/families and/or adults in Androscoggin, Franklin or Oxford County and surrounding towns. The BHH program is a community-based, integrated approach to providing care that addresses mental health, medical and other resource needs.

We are seeking people committed to the holistic approach to service provision, the ability to promote healthy lifestyles and wellness strategies & encourage the use of self-management techniques for mental health symptoms.

**$500-$1,000.00 sign-on bonus (dependent on caseload size) for new hires with 5+ years of experience in Case Management/Behavioral Health Home Work. Paid after successful completion of 90 days of employment.**

Position Summary:

As the Wellness Coordinator, you will work alongside a Peer Support, Nurse Care Manager and also BHH members, help the member manage and coordinate medical and mental health services, identify the resource to support goals for wellness and also facilitate educational group sessions when appropriate.

Essential Duties and Responsibilities:

  • Maintain an understanding of the core requirements of the Behavioral Health Home.
  • Provide community based services to clients and/or families.
  • Maintain commitment to the holistic approach in service provision for positive outcomes, providing care that integrates both physical and mental health.
  • Work collaboratively with BHH team members including Nurse Care Manager, Peer Support, Medical Consultant, and Psychiatric Consultant.
  • Complete comprehensive assessments of service care needs; Develop service plans with clients and/or families; and maintain accurate client files according to agency policies, procedures, and in compliance with law, licensing, contracting, MaineCare regulations, consent decree, and the rights of recipients.
  • Independently and effectively manage a caseload including scheduling of client appointments, meetings, and office time for completion of required paperwork.
  • Co-facilitate educational groups for clients and/or families.
  • Collaborate with external community based services and providers.
  • Able to use electronic health record system for documentation.
  • Perform other duties as assigned.

Qualifications:

  • 3 years of experience in the field of social/healthcare services and at least 2 years direct case management or behavioral health home experience preferred.
  • MHRT/C or provisional is required to work with adults and/or
  • Bachelor’s Degree in social services or related field is required (LSW preferred) to work with children.
  • The ability to: work independently as well as a member of a collaborative team, have a high-degree of professionalism and ethical standards; and strong written/verbal communication skills.
  • Must be comfortable with the use of technology such as electronic health record documentation.
