Health Affiliates Maine is seeking a clinical social worker (LCSW), clinical professional counselor (LCPC), marriage and family therapist (LMFT), or psychologist, licensed in the state of Maine, who ideally has some experience working with geriatrics population. The candidate will provide outpatient therapy services for a nursing home in Waterville 4-5 days/week.

The successful candidate will need to pass all background checks. The ideal candidate would be willing to be credentialed with all insurances and does require the ability to bill Medicare.

Nurse Care Manager

Nurse Care Manager is a member of the Behavioral Health Home Team, a program designed to integrate both behavioral and physical health treatment. The team also includes a clinical team lead, peer support specialist, wellness coordinators, PCP consult and psychiatric consult.

As a member of the team you will assist in establishing a plan of care that identifies and re-assesses cost-efficient and appropriate levels of care for members. The Nurse Care Manager will enhance collaborative relationships with multi-disciplinary teams and primary care practices, develop/find chronic health education materials for members, monitor member records and report to team regarding potential adverse outcomes and co-facilitate facilitation of group education on chronic disease management.

Qualifications:
Applicants must be a registered RN, licensed in Maine in good standing as a psychiatric nurse, certified by ANCC as a psychiatric and mental health nurse; an advance practice psychiatric and mental health registered nurse licensed as a NP or CNS who has graduated from a child and adolescent or adult psychiatric and mental health NP or CNS program. Must be certified in good standing.  Three years of experience in a PCP office or psychiatric setting preferred.

The preferred candidate will have at least 2 years of experience providing Targeted Case Management services to clients with mental health or behavioral issues and will be able to work independently. Qualifications:

  • MHRT/C or provisional is required to provide adult case management.
  • Bachelor’s Degree in social services or related field is required (LSW preferred) to provide child case management.
